Santiam Memorial Hospital’s Case Management Staff focuses on three primary areas of care to assist patients and families in the care, ongoing treatment and recovery:

The team is compromised of case managers who are committed to delivering personalized services to promote continuity and quality of care.

- Medical equipment and supplies
- Resources for alternate levels of care, (Assisted Living, Intermediate Care Facility, Adult Foster Care, Residential Care Facility. Skilled Nursing Facility, etc.)
- Arrange hospice care
- Resources for respite care
- Medication assistance programs
- Finding a local provider
- Respiratory and oxygen supplies
- Resources for the aging and disabled persons
- How to locate a local caregiver
- Resources for caregiver support
- Resources for financial support (Disability, Medicaid, Oregon Health Plan)
- Discharge planning following an acute hospitalization
- Arrange for Home Health Services
- Resources for grief or bereavement support programs
- Resources for support groups and counseling
- Assist with finding homeless shelters or shelters for domestic violence victims
- Resources for transportation
- Resources for abuse against the elderly/disabled or children
- Crisis resources
- Substance abuse programs
